if he did not put the woodruff key back in it it could have loosened up and (from the engine firing and the fly wheel wanting to stay still) could turned retarded enough to make it fire way to soon and cause the engine to run backwards

but the fly wheel would have to spin almost 270* to get far enough to run backwards

or it could have been advances more than 15* before tdc
